The Behavioral Health Clinician will provide counseling services to our patients under the Collaborative Care model. They will collaborate with other professionals to evaluate patients’ condition and to assess needs. Advocate for patients to resolve crises. Refer patient or family to community resources as appropriate. Investigate abuse or neglect cases and take authorized protective action when necessary. Monitor, evaluate, and record patient progress according to measurable goals described in treatment and care plan. Identify environmental impediments to patient progress through interviews and review of patient records. Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the patient.
