Elder Life Specialist Job Description:

Identify appropriate candidates based on clinical documentation and clinical assessment of needs for Dementia, stroke depression, anxiety delirium management orders triggers hearing and visual impairments. Assess mental status of patients visited using Cam tool during visits. Clinically assess need for counseling and education to patient (if applicable) and family at bedside Collaborate with other staff in the development and implementation of initialized therapeutic interventions incorporating the delirium prevention protocol. Collaborate with other staff in providing appropriate behavior management techniques for patients with dementia Educate families on delirium and counsel on techniques to manage behaviors associated with delirium Assist with counsel to families preparing to take patient home with signs of fluctuating delirium Document clinical impression and interventions used with patients seen in order to facilitate continuity of care among Elder Life specialists Assist in training and supporting volunteers as volunteer program develops Maintain activity supplies on the units
