The RN Case Manager will work as a care team member to support the needs of the clinics and assume responsibility for the ongoing case management of an assigned panel of patients.  As a case manager, the RN will coordinate with a multidisciplinary team that includes patients, families, physicians, medical assistants, social workers, counselors, and other clinic support staff to provide high quality care.

Required Qualifications:

Graduation from an accredited school of nursing. upon hire: Alaska Registered Nurse License. 2 years Professional nursing experience.

Preferred Qualifications:

2 years Case Management in a clinic or outpatient setting.
