Case Manager (RN)

Job Summary: Position is responsible for implementing, coordinating, and evaluating the physical and mental wellness of hospitalized patients. A case manager is a member of interdisciplinary team, working with the attending physician, specialists, and bedside nurses; the case manager helps to coordinate the patient's transition to post-acute setting. A case manager often works with nursing homes, home health agencies, medical-equipment providers, and insurance companies to seamlessly facilitate a patient's move to the next level of care. A case manager works closely with physicians, nurses, mental health, and other health care professionals, as well as family and friends of patients.

 

Job Duties:

Educate patient and family on the role of the case manager. Assess patient and family needs upon admission through the Discharge Coordination Evaluation (DCE). Identify needs based on discharge planning evaluation and begin establishing resources needed to meet patient and family goals.  Build rapport with patient and family. Document discharge plans and anticipated needs in electronic medical record. Identify potential discharge date and communicate discharge plan to IDT. Incorporate IDT recommendation into discharge plan. Make referrals to post discharge agencies according to patient preference. Motivate patient and families to actively participate in plan of care. Maintain a current and evolving discharge plan on all inpatients, starting within 48 of admission, that meets the needs of the patient and family throughout the course of the hospital stay. Ensure patient/family understanding of plan of care and are offer opportunities to participate in decision making. Deliver IM letters as per CMS expectations. Performs other duties as assigned by supervision.

 

 

Qualifications: 

Education:   Requires licensure in nursing or other related healthcare field (Bachelor’s preferred)

 

 

Training and Experience:

 

BLS required Work experience: Minimum two- years clinical experience. Demonstration of effective problem-solving techniques including conflict management skills Demonstration of excellent oral and written communication skills Excellent organizational and record-keeping skills, as well as the ability to manage multiple cases. Completes responsibilities in a timely manner through effective organization, setting priorities, delegation, and time management skills

 

Job Knowledge:  Knowledge of Conditions of Participation for Discharge Planning Knowledge of WRMC’s policies and procedures, regulatory and legal requirements and current health care standards and ensures compliance

 

 

 

Essentials:

Must be able to report to work fit for duty and free of any adverse effects of illegal drugs, medical marijuana, prescription medication, and/or alcohol.  Must be able to effectively communicate both orally and in writing with other individuals. Must be able to work independently, anticipate and organize workflow, prioritize, and follow through on responsibilities. Must be able to skillfully manage a high-volume caseload and to respond effectively to rapidly changing priorities

 

Physical Demands: Sitting, standing, walking, and lifting will occur for the majority of the shift. Must be able to lift 20 pounds. Must be able to sit/stand for extended periods of time. Fine motor skills. Visual acuity. Depth perception.
