Clinical Pharmacist

Interpret, compound, formulate and monitor all physician prescriptions; provide the best possible drug therapy to Home Service Long Term Care patients under the highest of professional standards. The pharmacist works closely with nurses and physicians to assure that patients are receiving quality care that is delivered in a safe and efficient manner. This requires specific knowledge in geriatric and pediatric medications.

Required Qualifications: 

Education to meet certification, license or registration requirement. upon hire: Oregon Pharmacist License upon hire: In Compliance with the Department of Human Services (DHS), criminal background check is required for this position, based on assigned duties. Fingerprinting may also be required. upon hire: In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties. 1 year Pharmacist associated experience or completion of a PGY-1 residency preferred
