Responsibilities + Conduct comprehensive, efficient, and sensitive intake and assessment, triage crisis needs and act as an advocate for customers with community based agencies, government agencies and others, modeling appropriate communication methods and successfully assist customers to obtain benefits and assistance to ameliorate the crisis. + Conduct timely, efficient and active internal and external referral processes and follow up timely on all referrals and document outcomes, determining appropriate next steps and appropriateness of referred services. + Determine need for direct assistance from Community Crisis Network emergency fund based on written criteria and ensure follow up steps are taken by customer as agreed to before recommending customer for subsequent emergency assistance. + Provide short term case management (90 days) and ensure support so customer's crisis is ameliorated. As directed provide long term case management and/or engage customer to long term case management, utilizing in-house resources. + Conduct outreach to engage partners at community sites and represent the Agency's vision and values, ensuring relationships are developed proactively and consistent with SCAP's strategic initiatives. Develop relationships with customers and their communities representing the Agency's community action philosophies. + Coordinate and work actively with partners and colleagues as a team to conduct case conferencing when appropriate and utilizing the family development process and solutions focused approaches, documenting outcomes on a timely basis. Qualifications Requirements + Bachelor's Degree in Human Services or related field with two (2) years' experience providing services to families and individuals. + Master's Degree Preferred. + Knowledge of community-based services, public assistance and other community resources and demonstrated skills in accessing these services appropriately. + Must have the ability to learn quickly the resources available in Schenectady County, and be able to appropriately refer in-house working with other program support staff to ensure full access to SCAP's services. + Knowledge and understanding of issues related to individuals and families in poverty. Ability to build upon the strengths of those we serve with diverse racial, cultural, social, educational and economic backgrounds, including high risk populations, to provide high-level services and lasting solutions to fit each individuals and family's needs. + Background check is required through the NYS Central Registry of Child Abuse. + Applicants must be fully vaccinated against COVID-19 and show proof of vaccination [or receive a first vaccination by their first day]. Any requests for an exemption will be reviewed by the SCAP Human Resources Director and determined on a case by case basis.
