Housing Support Specialist

Schedule: Mon-Fri 9am-5pm

Salary: $50,000 / year

Sign On Bonus: $1000; $500 upon completion of 30 days, $500 upon completion of 6 months

The Housing Support Specialist provides specialized housing services to individuals enrolled with the Behavioral Health Community Partner (BHCP) program or with an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan.

The Housing Support Specialist supports and collaborates with the BHCP teams which provide intensive care coordination and clinical care management for MassHealth members with complex medical and behavioral health needs. In coordination with Housing Program Manager, the Housing Support Specialist provides services, including assessment of housing related needs and navigation to other Tenancy Support Programs. For housed members who are at risk due to qualifying health conditions or are housing insecure, the Housing Support Specialist provides Pre-Tenancy Support Services, Tenancy Transition Services, and Tenancy Sustaining Services to resolve issues that impact continued tenancy. In addition, the Housing Support Specialist works with members who are experiencing homelessness to identify housing needs and preferences and helps them access housing.

Additional responsibilities include:

Helping enrollees identify preferences, assistance with budgeting or referral to aid programs, supporting access and completion of housing applications, and supporting follow up to applications.
Provides Tenancy Sustaining supports, include negotiating with landlords regarding accommodations, connecting to tenancy sustaining benefit programs, connecting to education regarding lease compliance, and connecting to legal resources or resources for successful community living.
Performs outreach to assigned Enrollees and verifies eligibility for the program, completes flexible services screening, verification, planning, and referral. Identifies the need for and coordinates Home Modifications that ensure the member's health, welfare, and safety, or enables the member to function independently in a community-based setting (i.e., installation of grab bars and hand showers, doorway modifications, in-home environmental risk assessments, training to use such supplies and modifications correctly). Uses evidence-based and promising practices that optimize care integration and effective communication across systems of care, and that align with rehabilitation, recovery, and wellness principles, such as the Housing First approach to care.
Uses recovery-oriented, person-centered, culturally competent, and strengths-based approaches to help members find, get, and keep housing.
Develops and maintains relationships with internal and external parties and agencies providing service to persons experiencing homelessness or who have at risk tenancies.
Performs other related duties, as assigned.
