Nurse Navigator (RN) Home Hospital

Summary of Position

 

located at Salem Hospital ED

 

The Emergency Department Nurse Navigator is responsible for accurate and efficient patient identification for the MGB Home Hospital. The ED Nurse Navigator identifies eligible patients and conducts clinical and social eligibility and safety screening while collaborating with the Home Hospital Admitting provider, the Home Hospital Case Manager, and the Administrative Coordinator. This role also communicates with the ED care team, the patient and family and the ED Case Managers at times to facilitate transfers to MGB Home Hospital.  This role is the patients’ first introduction to MGB Home Hospital because of this, the role requires a strong understanding of the program and the ability to present the program accurately, addressing any questions raised by the patient, family or care team.

The ED Nurse Navigator is present in the ED, proactively, case-finding by engaging in rounds and huddles, as well as independently identifying patients throughout the shift to maintain goals of Home Hospital growth and census.

 

Responsibilities

·        Attends huddle each morning with the Home Hospital team

Gathers appropriate clinical information to identify patients based on catchment area and plan of care Interacts with the Emergency Department interdisciplinary team and ED Care Facilitators to assist patient identification with the goal of successful transfer to MGB Home Hospital Collaborates closely with the MGB Home Hospital Admitting Provider when perspective patients meet eligibility criteria; then notifies the Home Hospital Case Manager Obtains consent from patient to transfer to MGB Home Hospital Communicates and collaborates with Administrative Coordinator and Administrative Manager regarding patient transfers Has real-time knowledge of capacity and capabilities of MGB Home Hospital Works with care areas to manage patient flow and placement to support Home Hospital transfers and census goals Utilizes analytical and critical thinking in decision making and judgment that is aligned with organizational objectives Serves as a role model, coach, and facilitator Promotes thorough collaborative and respectful communication Inspires and encourages teamwork Exhibits excellent and effective communication skills that promotes patient centered care, relationship building, and professionalism

Requirements

·        Current RN licensure in the state of Massachusetts

·        Minimum 3 years emergency department, home hospital, or inpatient experience

·        Varied experience preferred
