Principal Accountabilities:
Direct Patient Care: Assists physician and patient during visit.
Assess reason for visit and informs MD prior to visit
Ensures that all labs are charted in advance or at the time of visit
Assesses vital signs and overall wellbeing of patient, communicates with MD
Under the direction of the physician, conducts patients visits to do medication adjustments
Advocates for patients, family member
In collaboration with MD develops patient care plans and communicates with families. Adjust plans of care as appropriate.
Provides assistance and guidance to medical assistants in maintaining patient flow
Indirect Patient Care: Responsible for phone management of assigned patients.
Responsible for pharmacy contact, medication ordering and renewal
Reviews and appropriately disseminates labs based on acuity
Triages phone calls to determine acuity to assist in scheduling appointments
Reviews and assesses 24 hour and 30-day monitoring and disseminates to MD as appropriate
Liaison between patient and physician communicating patient updates, concerns, problems Facilitates interdepartmental communication between family and other pediatric specialists.
Complete school notes, forms, letters as needed
Education: Identifies educational needs of patient and families concerning alterations in health, the disease process, hospital plan of care, and discharge plan.
Coordinates/participates in community education programs. Organizes and conducts supportive counseling and education for patient and family groups with the social worker. Pursues and participates in formal and independent learning activities to enhance knowledge, clinical competency and professional development
Administration: Provides clinical data to obtain pre authorization and facilitate scheduling of cardiac catheterizations, MRI s and other procedures.
Ensure JCAHO, OSHA and HIPAA compliance for the practice
Knowledge of EPIC a plus
