SpringHill Home Health is seeking a Registered Nurse Case Manager to join our team. Flexible scheduleCompetitive pay + mileage reimbursement401K with Company match! Tuition reimbursement + other discounts Responsibilities Conducting patient assessments, re-assessments, resumptions of care (ROC) assessments, admissions, and discharges planning by utilizing the Outcome and Assessment Information Set (OASIS) used to plan careMonitoring, reporting and charting patient symptoms to medication and treatment plans, noting changes in patient’s condition, updating nursing care plans, recording supervisory visits, evaluating patient care and coordinate discharge planningSupervising Licensed Practical Nurses / LPNs and Certified Nursing Assistants / CNAsInstructing LPNs in special procedures and care plan complianceCommunicating with collaborating physicians or specialists regarding patient careProviding advice and emotional support to patients and their family membersAttending and participating in multi-disciplinary patient case conferences, staff meetings and in-service education programsParticipating in the rotating on-call schedule for after-hours ROC assessments and admissions, if necessary Qualifications RN license in good standingMinimum of one (1) year or recent home health, private-duty skilled nursing, hospice, hospital or case management experience preferredFamiliarity with electronic medical records / MatrixPOC knowledge a plus and general usage and function of a computer desktopProficiency in accurate and timely documentationStrong problem solving and critical thinking skillsAbility to build rapport with patients and familiesReliable transportation, valid driver's license and current auto insurance About our Line of Business At Springhill Home Health and Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
