And above all, it means exceptional care, simply delivered — and we couldn’t do it without you. **Make a Difference** The Registered Nurse Ambulatory functions as a member of the REACH team, conducting duties such as: phone triage, patient education, care coordination, referrals, scheduling, navigation, medication refills, rooming patients, lab draws, giving medications/injections, chronic care management and other related tasks as needed. The REACH (Resources to Evaluate and Advance Comprehensive Health) program is designed to provide primary care services to a cohort of patients within CHNw that have complex health and social needs. The position is located on the north side in Indianapolis, Indiana. **Exceptional Skills and Qualifications** -Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. -Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC). -One year or more of nursing experience required. -Phlebotomy experience preferred. -Previous medical office/outpatient experience preferred. -Triage experience a plus. -Excellent communication skills. -Excellent team skills. -Compassionate and caring. -Accountable and self-directed. -EPIC experience a plus. **Why Community?** At Community Health Network, we build teams that deliver exceptional care through empathy, communication and collaboration.
