Job Summary: Assist in the assessment, treatment of the patient, develop patient care plans, specially examining the significant social, legal and emotional factors as they relate to health problems related to patients/families facing a terminal illness and end-of-life needs. Active member of the interdisciplinary team. Source of supportive counseling and education surrounding end-of-life to both individuals, and families. Have a working knowledge of the community’s social, financial, legal and health resources. Understanding of federal and state services for the client. Have the ability to work a flexible schedule.
Qualifications: Master’s degree in Social Work , at least one year experience in hospice, medical social work, or other in a healthcare setting preferred. On going participation in continuing education.
