Summary:
Responsible for discharge planning for patients with complex issues requiring assistance with support systems, financial resources and other resources needed to enhance patient functioning. Works closely with Care Manager, Physicians and Nursing Services in implementing patient care and discharge. Provides support that improves patients/families ability to cope with hospitalization, illness, and resultant psychosocial problems.

(e.g., reliability). • Acts as a role model for other employees within the department or organization. • Adheres to NLBHH professional appearance policy / dress code. • Maintains and exhibits a professional, positive attitude towards co-workers, work, and organization. • Demonstrates a commitment to co-workers, job duties, and organization (e.g., team player). • Maintains effective working relationships with community agencies and facilities • Daily/weekly department and interdisciplinary team meetings • Able to locate and comply with NLBHH/NLH Policies. • Provides excellent customer service, both for internal and/or external customers. • Responds to phone calls, voice mail messages, and emails in a timely manner. • Participates in committees as assigned. • Demonstrates positive inter/intradepartmental communication. • Promotes and fosters patient-centered care. • Builds and maintains effective partnerships with external providers of care (NH, Home Health, PCP's, DHHS, etc.). • Completes HealthStream training as required for current year. • Adheres to "EMHS Code of Conduct" and follows all applicable compliance policies/regulations. • Works safely and follows all safety policies; reports incidents or hazards immediately. • Takes ownership of problems and proactively seeks out solutions. • Screens admissions for patients with complex discharge planning needs • Collaborates with Care Manager and Multidisciplinary Team to plan and implement complex patient discharge. • Supports and promotes a safe work environment by reporting incidents timely. • Is aware of and participates in PIP projects. • Engages in quality initiatives and reporting specific for the department. • Models and fosters clinical reasoning and critical thinking to drive a culture of safety within the department. • Ensures safe transitions of care as evidenced by discharge handoff ensuring safe coordination of care by communication within the interdisciplinary team; outpatient providers of care and system care coordination. • Seeks out ways to reduce operating costs or increase efficiency within role/department. • Acts in a cost-conscious manner with regard to use of supplies. • Aware of budget and department participation in conserving finances. • Identifies barriers to discharge planning and facilitates a timely resolution for safe discharge. • Aware of and supportive of departmental productivity model. • Supports and embraces change in job and organization. • Proactively seeks out solutions to problems. • Continually develops skills and/or job knowledge. • Participates in skills training opportunities available to him/her. • Actively engages in conversations, planning and implementing department plans as assigned. • Works with team to develop and improve customer experience. • System thinker i.e. "big picture thinker" within the organization and NLH. • Seeks opportunities to further knowledge in career field i.e. Social Work education; leadership development; etc. • Develops and maintains positive relationships with community resources and potential referral sources. • Assists internally and within the community to navigate healthcare and community resource options.
