The Opportunity:


The Resident Care Coordinator is responsible to the Executive Director or the Director of Care.  They  ensure and support the implementation and evaluation of the resident care services in accordance with applicable provincial standards, guidelines, and regulations to ensure that the highest standard of resident care is delivered and maintained at all times. They promote teamwork and a culture that demonstrates the guiding principles of AgeCare; Trust, Respect, Teamwork, Quality and encourages practices that respect resident, family and staff rights, individuality, and diversity.


What Is In It For You:

Provides support to all Care staff to attain and maintain quality care within a person-centered model of care.

Role models AgeCare Vision, Mission and Values.

Reviews trends on all incident reports and medication incident reports and ensures prompt corrective action to prevent recurrence.

Participates in Provincial Continuing Care Health Service Standard compliance processes and completes action plan development with follow-up.

Ensures Critical / Reportable Incident and all allegations of Resident abuse (PPC investigation processes) are reported in collaboration with the Executive Director or Director of Care and the Community RN Case Manager.

Ensures Residents are admitted in accordance with AgeCare policy and provincial standards.

Is accountable for overseeing the admission process of residents in collaboration with the Community Placement Coordinator for available beds, strategically selecting potential residents while balancing multiple variables (safety, roommate suitability, space considerations).

Ensures that bed vacancies are filled within 6 working days and that the facility is kept fully occupied.

Ensures Resident Annual Care Conferences are scheduled.

Keeps the Executive Director or Director of Care and facility management team apprised of any emerging issues.



What You Will Need:

Minimum LPN Provincial Certification required.

Must be fluent in to perform work in English, both orally and in writing

At least 5 years’ experience in Health Sector (or other Nursing designation) in continuing care setting, demonstration of leadership abilities with strengths in supervision, leadership, judgment, and interpersonal skills.

Experience in caring for Residents with Dementia.

Salary Range: $74,000.00 - $82,000.00 per year

Salary placement based on qualifications and experience.
