245D Intake Specialist

Are you looking to make a meaningful impact on the lives of individuals being supported in a home-and-community-based service setting? Do you have experience with 245D admissions and writing service plans? Pathways to Community (PTC) is seek qualified individuals to fill our Intake Specialist role. In this role, you will market the openings within the company and navigating referral sources in locating service options available. You will cultivate relationships with county and contracted social workers/case managers in an effort to feed any service openings within Pathways to Community.
Position Responsibilities :
+ Manage the initial interactions with referral sources to compile and record data to verify appropriateness for 245D services within PTC.
+ Maintain working knowledge of the population in each department and program to effectively determine appropriateness for placement.
+ Cultivate relationships with referral sources, Support Team members, and family members. Be available for questions and respond in a timely manner both during the referral process as well as during the admission process.
+ Keep accurate and timely records regarding denials and admissions.
+ Explain regulations, policies, and procedures as needed to referral sources, Support Team members, PTC staff, etc.
+ Plan and prepare for all admission meetings by collaborating with Support Team, Applicant, as well as PTC Director, General Manager, and Manager.
+ Prepare admission documents such as consents, releases, service plans, abuse plans, behavior management plans, etc. as required by 245D.
+ Attend and lead all admission meetings. Obtain signatures on all documents. Finalize all service, abuse, and behavioral management plans and protocols before handing over to the assigned department.
+ Complete the Shelter Verification form and submit to the county of financial responsibility.
+ Complete the 6790 and/or cost proposals and provide to the social worker/case manager. Tracking initial service agreements and ensure they are accurate.
+ Set up employees with the necessary tools to train on new admits prior to service initiation and then follow up with any new information or tools obtained at admission.
+ Attend 45-day meeting to ensure collaboration of services existed in the first 45 days of service.
+ Work collaboratively with Compliance or the department’s management team on recommended practices for admissions and assessments during the first 45-days of service.
+ Keep informed regarding pending industry changes, trends, or best practices. Make recommendations to the Leadership Team as needed for revisions to policies, forms, or operating practices.
+ Assist with licensing, audits, and support with overall compliance with 245D and CRS licensing as needed.
+ Other duties as assigned.
Education and Experience :
+ Must meet requirements to be considered a Designated Coordinator under MN Statute 245D.
+ Extensive knowledge of MN Statutes 245A, C, and D.
+ Experience coordinating referrals and admissions in a program licensed under 245D or similar type license.
+ Ability to use a computer, Microsoft office, and other software.
+ Demonstrates innovation by being creative and altering thinking to develop new ideas for and answers to work-related problems.
+ Ability to read, write and speak English at a level that meets the performance requirements.
+ Strong receptive and expressive communication skills.
+ Must possess a strong attention to detail and a high level of independence in managing role and tasks assigned.
+ Possess a valid driver’s license, proof of liability insurance, and a satisfactory driving record.
+ Ability to pass criminal/registry background check.
Location and Schedule:
+ As a member of our administrative team, you will have an office at our main building located in St. Paul.
+ While your work schedule is primarily Monday through Friday during business hours, it is subject to flexible and varied hours, depending on the needs of the company.
Compensation and Benefits:
+ Hourly Range: $27.00 - $29.00 DOQ.
