Details of the position include: Location: Danvers, MA Program: Eliot - Program for Assertive Community Treatment (PACT) Schedule: Mon-Fri, 8a-4p with a standing 12p-8p shift Start Date: Two weeks from offer Salary: $28/hr *MUST have a Massachusetts license and reliable transportation as you will be doing home visits for clients. Duties and Responsibilities (these are a general summary and not all inclusive): Provide case management services as part of a multidisciplinary team Develop treatment plans with consumers. Act as a primary case manager to consumers in accordance with Eliot’s policies and PACT standards. Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner. Assess client's functions, strengths and needs. Develop therapeutic connections with each client and provide clinical interventions.
