85 yr old male with dementia, and needs COMPLETE assistance Start at 3pm and end at 8pm (5 hours) Mon-Fri Needing help w/ALL ADLS, uses a wheelchair. He is on a dysphagia diet, needs thickened liquids. Can become agitated at times. Needs to be redirected and reoriented.
