Responsibilities
Promotes and maintains the continuum of patient care from the hospital or MD Office to home or alternate site setting specific to patients in need of Infusion therapy or enteral nutrition.
Collaborates with the multidisciplinary team and servicing home health agencies to facilitate quality, cost effective services in the home, while maintaining continuity of care and achieving positive patient outcomes.
Performs sales duties in and around assigned hospitals, including all relevant clinics and physician offices.
Coordinates entire referral, including insurance verification, home health scheduling, discussing financial responsibilities with patients, working closely with operational staff, etc.
Gives high priority to patient and referral source satisfaction.
Identifies issues and concerns the patient and/or family may have and assist in issue resolution.
Follows established administrative and human resources policies and procedures to always ensure compliance.
Attends educational in services, departmental staff meetings, and mandatory training and meetings as required.
Maintains patient information and records confidentiality.
Previous managed care/insurance experience preferred.

Able to interact and communicate with multiple sources, including the institution staff, physicians, home health servicing companies, community agencies and resources, patients, their representatives and/or family.
