Demonstrate ACO subject matter expertise internally and externally.Create and distribute educational materials for providers and staff on ACO workflows and initiatives.Answer program questions from staff related to general ACO questions and performance. Attend meetings with external ACO partners and be available to assist with their requests, ensuring timely and accurate follow-up on assigned tasks.Collaborate with internal teams to align on ACO performance improvement strategies.Data Management and Reporting:
Collect, analyze, and validate performance data from various sources (EHR, claims data, etc.)Prepare regular and ad hoc reports on key performance indicators (KPIs), including quality measures, beneficiary attribution, Annual Wellness Visit completion, risk scores and readmissions.Maintain up to date performance trackers and dashboards to monitor progress.Own the data requests from ACO, such as up to date provider and facility lists.Performance Monitoring:
Identify gaps in value-based metrics (quality, AWV, documentation) and other performance indicators and escalate findings to leadership. Qualifications Associate or Bachelor’s Degree preferred, preferably in administration, business, or healthcare related field (or equivalent work experience).5+ Years of experience in healthcare operations, with a focus on primary care preferred.Demonstrated ability to work with healthcare data to support operational and performance goals.Prior experience with supporting ACO providers a plus.Familiarity with value-based care models, ACOs or healthcare quality programs.Strong analytical and problem-solving skills, including advanced Microsoft excel ability.Proficient in systems including Microsoft Office, EHRs, Power BI and other data platforms.Strong verbal and written communication skills.Excellent organizational and time management skills.Ability to work independently as well as part of a team.Ability to comply with HIPAA guidelines.Ownership mentality that shows up as taking action, thinking creatively, and finding iterative ways to improve.Comfortable with ambiguity and change while simultaneously building new processes and revising existing ones. OverviewWe are seeking a detail oriented and motivated individual to join our team as an ACO (Accountable Care Organization) Performance Specialist. This role is ideal for candidates interested in value-based care and healthcare performance management.
