Ebenezer-Lake City Care Center TCU LTC offers a robust benefits package and wages ranging from $16.04 - 17.89 / hour.

 

We are seeking an Activity Assistant to support our adult residents in our Skilled Nursing Facility in Lake City, MN! This role assists with implementation of quality leisure/social events program designed to assist residents in maintaining their personal potential both physically, socially, and mentally. This position will also drvie company van/bus for resident outings.

 

This patient support schedule includes;

Responsibilities/Job Description Assists with leisure programs and activitiesLeads social events and activity programsCommunicates and coordinates activity/leisure programs with all departments Gathers and transports residents for programs and activitiesAssists other departments with special events and family functionsDocuments as requiredOffers suggestions and ideas to improve the events program to the Director of Active LivingAssists in meeting the psychosocial needs of the residentsEncourages residents participationReports change in resident attitude, mood or physical status to appropriate staffPromotes residents independence by offering choices and fostering self-help skillsAssists residents in developing relationships and effectively socializingAssists with virtual visits upon recommendations of Active Living Coordinators and Active Living DirectorAssists in implementing pet visits based on recommendations of Active Living Coordinators and Active Living DirectorAssists Active Living Coordinators in maintaining compliance with state and federal regulations regarding medical record documentation and participationAssists in completing MDS, Assessments, Care Plans, and Progress notes for Active LivingCompletes and updates participation recordsGather information from other team members as it relates to residents' needsDrives company van/bus for resident outingsReviews and completes Vehicle Requirements - Transporting Residents Checklist prior to each resident outingMaintains a log of transportation services and tasks performed throughout the driving periodResponsible for the safety and welfare of passengersPractices sound, safe judgement when performing transportation services including but not limited to: fire/disaster procedures and infection control proceduresRecognizes and acts appropriately on safety hazards and incidentsFollows all laws regarding operating a motor vehicleImmediately reports any traffic violations or accidentsOther duties as assigned and directedResponds to resident and building emergencies per building policies and proceduresAdheres to applicable rules and regulationsTakes photos of new admitsAssists with transportation of residents to and from dining areasPerforms environmental and other responsibilities as assigned (order and cleanliness) Qualifications

 

Required:

0-1 years of experienceCurrent Driver's License (in good standing)Working knowledge of basic Microsoft applications

Preferred:
