Position Overview:
We are seeking an experienced and compassionate Acute Care Nurse Practitioner (ACNP) to join our inpatient specialty Cardiothoracic Transplant team. The ideal candidate will have a strong background in acute care settings, cardiology, thoracic surgery, or transplant medicine, and a passion for delivering high-quality care to critically ill patients.

Key Responsibilities:

Provide comprehensive care for patients in the cardiothoracic transplant unit, including assessment, diagnosis, and management of acute and chronic conditions. Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients undergoing cardiothoracic procedures. Perform advanced clinical procedures within the scope of practice, including central line placements, intubations, and other invasive interventions. Monitor patient progress, interpret diagnostic tests, and adjust treatment plans as necessary. Educate patients and their families on transplant processes, post-operative care, and lifestyle modifications to support recovery. Participate in daily rounds, team meetings, and case conferences to discuss patient care and treatment strategies. Maintain accurate and timely documentation in electronic health records (EHR).

Qualifications:

Master’s degree in Nursing with a focus on Acute Care Nurse Practitioner program from an accredited institution. Current state licensure as a Nurse Practitioner with Acute Care certification. Previous experience in acute care, cardiology, thoracic surgery, or transplant medicine preferred. Strong clinical assessment skills and the ability to make critical decisions in high-pressure situations. Excellent communication and interpersonal skills, with a commitment to patient-centered care.
