Administrative Assistant-Care Management, Full Time

Job Summary:

Primary responsibility is to provide administrative support and assistance to the Care Coordination Supervisor’s for various Adult Care Management, Children’s Care Management and Local Care Management activities.

Duties/Responsibilities:

Maintain discretion with sensitive, confidential information concerning Physicians, employees and patient related material, as well as information contained in minutes, letters, and other documents. KRONOS Answer phones and manage care management mailbox Registering and Discharging clients Completing monthly epaces to monitor for HARP and Medicaid eligibility Ordering department supplies; managing office vendors Maintaining department books, including tracking and reporting service dollars and petty cash Assist with preparatory activities and coordination for County contracts for LCM, billing, data collection, reports, data entry, County funding tracking and reporting. Information collection, communication follow-up, organizing and maintaining files as well as coordinating various paperwork associated with projects. Compose correspondence, memos, reports and minutes as instructed or required. Perform filing and record-keeping to allow for easy retrieval of information in a timely manner. Report in Census, CAIRS, and County Survey Monkey. Be able to navigate and utilize information using MAPP, E-PACES, PSYKES, CAIRS, UAS, KRONOS, EPIC, MEDITECH, and NETSMART. Process mileage vouchers Maintain staff files and competencies, schedule CPR and other mandatory trainings Maintain new employee folders and coordinating with HR on Criminal Background Checks Support efforts to comply with all regulatory agency requirements (e.g. DOH, CMS, OMH, CDC, Homeland Security and Joint Commission Standards) Maintain form drawers-update with new revised forms Help Desk Tickets-submit and coordinate Maintain on-call schedule Demonstrate supportive and understanding communication and behavior when in contact with guest, (patients, visitors, clients, physicians, employees, volunteers, etc.), always displaying positive guest relations. Perform other duties as directed by supervisor.

Required Skills/Abilities (Unit Specific):

Professionalism and business acumen appropriate for working with guests, physicians, employees and volunteers. Ability to create superior patient experiences while generating positive guest relations in an office setting. Proficient in standard computer programs, including Microsoft Office Suite, with excellent typing skills. Excellent verbal and written communication skills, as well as active listening skills. Excellent time management skills, organizational skills, and ability to handle several tasks simultaneously.

Education and Experience:

High school diploma or equivalent required Previous office experience preferred

Working Conditions:

Extended periods of sitting, using standard office equipment including, but not limited to computers, phones, scanners, fax machines, printers, etc. Requires attendance of seminars, workshops, conferences when appropriate. Requires leading and/or attending staff and executive meetings. Occasionally requires assembling, using, and/or maintaining department equipment. Occasionally requires prolonged periods of standing and walking with the ability to lift, push or pull up to 15 lbs. using proper body mechanics. Actively participates in Quality Assurance programs and CQI, in-service programs; completes department specific requirements/competencies; attends all mandatory classes as required; participates in meetings and committees as assigned. Complies with DOH requirements for annual health assessments and required immunizations.

Pay Range: $16.48-$18.13/hr

Employee salary is based on commensurate experience and other qualifications.
