Admission Coordinator II - (RN / LPN)
To develop relationships with referral sources and work with internal teams to promote and maintain census. To assist Social Service Department in meeting the needs of the residents and families of the care center. Every 4th Weekend rotation Ability to process and complete admissions. Provides pre-admission counseling, information and tours. Clinically evaluates referrals to ensure all needs can be met from a medical perspective Facilitates the intake process and secures primary care provider Reviews EPIC for proper documentation and orders Contacts pharmacy as needed to verify cost of meds Determines need for equipment and supplies and coordinates with internal team Facilitates PAS Screening and Level II as needed Coordinates admission process with resident and resident representative, community providers and staff as appropriate Sets up interpreter services as needed Assures completion of admission paperwork Informs residents and resident’s representatives of financial responsibilities, payment options and agency contacts according to the state and federal regulations Builds and maintains relationships with referral sources and assists with marketing and outreach activities as appropriate Maintains internal and external waiting lists as appropriate Verifies necessary payer source(s) as appropriate Coordinates room change process as appropriate Ability to promote professional working relationships within Ebenezer/Fairview and with outside referral sources Tracks referral sources and other information as requested Works with internal and campus team to maximize facility occupancy Refers inquiries to appropriate agencies as necessary Conducts outreach through campus and facility events Participates in Fairview TCN meetings as appropriate Provides internal training regarding admission process as needed Ability to assist residents with various tasks Assists residents with mail, finances, and MA as appropriate Notifies residents and resident representatives of room changes and completes necessary documentation Assists residents in ordering personal items (i.e., clothing). Assists residents with securing personal belongings Advocates for residents Offers support to residents and families Ability to assist Social Service team with various tasks Oversees care conference rescheduling. Tracks advance directive and legal appointment documentation Oversees resident storage letters Provides support to social worker as identified by supervisor. Completes and follows up on missing property reports Assists with follow up on grievances as appropriate Ability to participate in Vulnerable Adult investigations and ability to complete necessary documentation and reports. Requirements: Vocational/Technical Training or Associate Degree Licensed Practical Nurse (LPN) or Registered Nurse (RN) One or more years experience Preferred: Bachelor's Degree: In Medical or Human Service Field Three or more years experience Two or more years experience in LTC
