This role reports to the Clinical Leader. **Current Help at Home employees only will be considered for this position.** **_Duties/Responsibilities:_** + Supervise a team of nurses responsible for patient admission and discharge + Collaborate with Client Intake team to determine readiness of Home Health referral + Collaborate with Recruiting team to ensure timeliness of patient admissions + Assign admissions and discharges to team as appropriate + Complete comprehensive audits as determined to ensure regulatory compliance + May be required to complete admission or discharge visits as needed + Coach team on where to identify resources for home health patients + Train and mentor new Admission and Discharge Nurses + Identify potential marketing opportunities and attend events or assigns to team as needed + Responsible for performance evaluation and management of team + Perform other related duties as assigned **_Education and Experience:_** + Graduate of an accredited nursing school.
