Primary responsibilities include:
Communicating with the hospital case managers, Rehab and hospital services, community physicians, and various resource agenciesCoordinating services for patients discharging home from an acute care settingContacting physician offices, obtaining various services/orders (ie: DME, infusion) as needed for the homecare clinicians and ensuring referral information/orders are completeDocumenting patient information into our EMRWorking with various departments to coordinate a smooth transition to homeKey skills required:

Pay Range: $31.50 - $42.80
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
