Admissions Assistant
This Admissions Assistant position with Ebenezer assists the Skilled Nursing Facility (SNF) Corporate Admissions Department with the admissions process including insurance verification, communication, and data entry. This administrative role will support a smooth and efficient process to admit patients to our TCU/LTC units in Minnesota. Position is 16 hours in two weeks, working every other weekend. Ability to process and complete pre-admission tasks: Comprehensive understanding of SNF stays per Medicare, Medicaid, and HMO Insurance Contracts. Responsible for prior auth for a SNF stay, as required by ins contracts. Required to check Medicare and/or HMO insurance eligibility and notate number of remaining skilled days, as applicable. Retrieves records from EPIC for proper documentation and orders. Communication with the SNF and other agencies to ensure a smooth transfer to the SNF from the referral source. Coordinates with internal team at each Ebenezer SNF site to ensure all appropriate orders and equipment are in place prior to admission. Facilitates PAS Screening and Level II as needed. Communicates Change of Status (COS) with each site on all admissions and readmissions. Maintains waiting lists. Verifies necessary payer source(s). Coordinates room placement in SNF as delegated. Initiates the EHR for each admission. Assists with admission order entry i.e.: medications, diet, demographics, etc. as directed. Ability to promote professional working relationships within Ebenezer/Fairview and with outside referral sources: Tracks referrals and other information as requested. Works with internal and campus teams to maximize facility occupancy. Refers inquiries to appropriate agencies as necessary. Ability to assist Ebenezer SNFs with various tasks: Intake prospect calls and schedules tours with personnel onsite. Processes necessary paperwork and tasks as directed by the Director of SNF Admissions. Maintain regular communication with those on the wait list. Performs all other duties as assigned and directed. Required Computer skills Preferred Associate Degree 1+ years experience
