Admissions Coordinator
Professional Summary: An organized and compassionate Admissions Coordinator with over a year of experience in healthcare and customer service. Key Responsibilities: + Coordinate admissions for homecare, hospice, and skilled nursing facilities. + Obtain pertinent pre-admission documentation from referral sources to ensure the agency can adequately care for the patient. + Review clinical documentation to determine the level of care needed for patients. + Verify physician orders for accuracy and completeness. + Develop and maintain relationships internally within the organization and with external community partners. + Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families to ensure a smooth admissions process. + Provide excellent customer service to patients, families, and healthcare professionals both in-person and over the phone. + Maintain accurate records and documentation related to patient admissions and care. Qualifications: + High school graduate or equivalent. + CNA/MA preferred, but not required. + Demonstrate excellent customer service skills, both in-person and over the phone. + Ability to work independently and as a part of a team. + Basic computer knowledge. + Demonstrate excellent organizational and analytical skills. + Ability to multitask in a fast-paced environment. + Good verbal and written communication skills. + Minimum of 1 year experience in healthcare. + Knowledge of Skilled Nursing, Homecare, and Hospice criteria. + Minimum of 1 year experience in customer service or call center. Shift: Monday-Friday, Rotating weekly schedule 8:30-5:00 / 9:30-6:00 / 10:00-6:30, One scheduled weekend every 4-6 weeks Pay and Benefits The pay range for this position is $19.00 - $19.00/hr.
