As a Case Manager-LPN you are responsible for facilitating resident admissions and discharges, and ensuring a seamless transition throughout their journey with us. You will work with our residents and their families as well as internal/external partners to ensure residents receive the highest quality of care and services.

Responsibilities The LPN conducts pre-admission assessment of prospective residents.The LPN develops and ensures continuity of dialogue with hospitals/social workers in community and outlying services regarding availability within community.The LPN will coordinate with social services department on discharge planning. Qualifications Current state licensure as an LPN, in good standingPrevious admission/discharge experience preferredAble to read, write, and communicate in English
