SUMMARY
The Admissions Nurse (a) receives inquiries, conducts tours, coordinates admissions of residents into the facility and completes specific admission assessments; (b) assists the Director of Nursing in achieving budgeted census and works closely with other departments in regards to admissions, transfers and discharges.
PRIMARY RESPONSIBILITIES
1.Receive and respond to all incoming calls and in-person inquiries for admission to the facility to ensure that occupancy levels are maintained as needed.*
Provide appropriate follow-up to prospective residents/families, including phone calls, written correspondence and mailing of information. *Conduct marketing tours for prospective residents/families as requested to ensure that the residents’ needs can be met. *Maintain accurate records of inquiry and lead tracking, follow up, waiting list management etc. through the designated, computerized system. *Provide input to assessments, cost analysis and projected length of stay of potential new admissions, including hospital or site visits if necessary. *Consult with interdisciplinary team members regarding prospective residents with extraordinary needs. *Coordinate all of the needed services and supplies for residents prior to admission. *
2. Contact hospital social workers and/or nursing staff and gather accurate information on all admissions from the hospital or other admitting facilities in a timely manner. *
3. Maintain occupancy as designated. *
Establish and maintain effective working relationships with referral sources and outside agencies and organizations to market site services, to realize efficient and timely resident placement, and to participate with the various community constituencies to achieve mutually beneficial results. *4. Perform admissions assessment responsibilities. *
Prepare face sheet for the medical chart. Transcribe all admission orders into the Electronic Medical Record (EMR). *Assist with projects, audits, etc. as instructed by Nursing Administration.5. Provide accurate assessment, observation and documentation of resident care upon admission. *
Meet and welcome new residents/families on arrival at the facility. Assist nursing staff in orienting residents to their rooms. *Assure that residents are receiving appropriate care, per their assessment and Care Plan. *Communicate with Nursing staff and all other campus departments to facilitate quality patient care. *Coordinate nursing discharge functions including discharge plan of care and discharge summary. Meet with resident and or responsible party when available to review discharge instructions and medications prior to discharge. *Refer questions and concerns of residents/families to appropriate facility department managers.6. Ensure compliance with all Sholom established policies, procedures and practices with regards to the admissions process. *
7. Supports various other Nursing responsibilities as needed.

JOB SPECIFICATIONS
Education, Experience and Credentials
Graduate of an approved school of nursing and currently licensed as a LPN in the State of Minnesota required.Minimum of two years’ experience working with geriatric patients and one year of care plan development experience preferred. CPR Certification required.

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB
Incumbent must be able to:
Perform medium work, lifting up to 50 pounds, and occasionally lifting/carrying of objects weighing up to 25 pounds.Push/pull people and equipment up to 50 pounds a distance of several feet.Twist, turn, bend, stoop and reach frequently in the performance of client assistance responsibilities.Effectively communicate, including following oral and written instructions, in order to communicate with people inside and outside the organization.Able to effectively use office equipment to review and create information.Work at a desk for extended periods of time and perform long hours of work at a computer. Frequently stand and walk to move around office/building repeatedly throughout the day.Able to work a flexible work schedule, nights and weekends as required including attending events/meetings as needed.

Use telephone, computer, and standard office equipment Use standard nursing equipment such as: thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, IV equipment, suction machines, nebulizer machines, and oxygen equipment.
WORK ENVIRONMENT
The work environment is primarily indoors.
