Glenburnie Rehabilitation and Nursing Center has an exciting opportunity for a Registered Nurse or a Licensed Practical Nurse to join our team as an ADMISSIONS NURSE to facilitate the admittance of residents to the facility- aiding in the assessment, implementation and evaluation of resident care- and acting as a liaision between Interdisciplinary team members.

12pm-8pm or 11am-7pm

Responsibilities:

Collaborate with resident, physicians, nursing staff members, therapists and social services department to ensure seamless transfer to the facility Communications information effectively to physicians, supervisors, Administrator and any other pertinent personnel in a timely manner Ensuring all aspects of admissions are complete including, but not limited to, completing admissions orders/assessments, completion of electronic medical record charting and developing a baseline plan of care. Providing a seamless transition with the scheduling of appointments, processing pharmacy orders, etc to maintain continuity of care. Assist in admission planning and pre-admission screening. Provide support to charge nurses, residents and other team members to exceed resident expectations while providing exceptional customer service.

Qualifications:

Active and unencumbered LPN or RN license  Minimum of two (2) yeas experience in long-term care Strong computer skills  and experience with PCC Excellent verbal and written communication skills Must be devoted and compassionate about their chosen career
