Responsibilities Complete initial and ongoing comprehensive assessments of patients and familiesInitiate patient plan of care and reevaluate as necessaryUtilize health assessment data to determine nursing diagnosisAdminister medication and treatment as prescribed by the physician in the plan of careCounsel the patient and family in meeting nursing and related needsComplete and maintain accurate clinical notesCommunicate with the physician and participate in interdisciplinary group meetings to meet the medical needs of the patientEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation1 year nursing experience required, 3+ years preferredExperience in home health or a similar setting preferredCommitment to clinical and documentation excellence About our Line of Business At AT Home Care & Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
