Admissions Specialist / Visit Facilitator Position Summary Pod Health is seeking a highly organized and detail-oriented Admissions Specialist / Visit Facilitator to join our dynamic care team. This dual-role position focuses primarily on managing patient admissions and post-visit processes, while also supporting patient-provider interactions as needed. The ideal candidate will possess strong administrative skills, a passion for patient care, and a proactive approach to operational excellence. Primary Role: Admissions Specialist Objective: To oversee all post-visit processes, ensuring timely and accurate follow-up, service enrollment, and documentation management. Core Responsibilities: + Post-Visit Review: + Review provider notes and patient records in EMR + Identify required follow-up actions, including service enrollments and care updates. + CRM Management: + Create and manage CRM opportunities for various service lines. + Track and update patient progress and service outcomes in CRM. + Data Management: + Ensure all patient data is accurate and up-to-date. + Verify insurance, service eligibility, and demographic information. + Documentation Compliance: + Review and submit all required patient documentation. + Ensure compliance with healthcare regulations and company policies. + Service Coordination: + Coordinate with internal case managers and service specialists. + Communicate effectively to resolve outstanding tasks and service delays. Secondary Role: Visit Facilitator Objective: To ensure seamless patient-provider interactions by managing appointment preparation, technical setup, and visit coordination. Core Responsibilities: + Pre-Visit Preparation: + Confirm appointments by contacting patients 30 minutes prior to their scheduled visit. + Assist with Telehealth setup, ensuring proper video and audio functionality. + Appointment Management: + Notify providers of patient readiness and share key visit details. + Manage cancellations and reschedule appointments in coordination with the Scheduling team. + Patient Support: + Assist patients with troubleshooting technical issues before and during visits. + Ensure patients have completed all required consent forms. + Real-Time Problem Solving: + Handle last-minute changes such as cancellations or no-shows. + Utilize waitlists to fill open slots and maintain provider schedules. Qualifications and Skills Required: + Education: + High School Diploma or GED required; Associate or Bachelor’s degree in Healthcare Administration, Public Health, or related field preferred. + Experience: + 1-2 years of experience in healthcare administration, patient admissions, or care coordination. + Experience with Salesforce, Advanced MD, CCIQ, or other patient management systems is highly desirable. + Skills: + Strong attention to detail and data management skills. + Excellent communication and customer service abilities. + Ability to multitask and manage competing priorities in a fast-paced environment. + Problem-solving skills with a focus on patient-centered solutions. + Technical Proficiency: + Proficient in Microsoft Office Suite (Word, Excel, Outlook). + Experience using healthcare CRM and EMR systems. + Comfortable with telehealth platforms like Doxy.me, Zoom.
