Adult Care Manager- Care Management, Fulton

The Adult Care Manager provides a range of client centered services that link clients with health care, behavioral health treatment, and other psychosocial services as dictated by the plan of care.  

Provides a network wide focus to the coordination and management of client care.  Works closely with family members when appropriate and all service providers to ensure the client’s needs in medical, behavioral, and psychosocial domains are met and maintained.

Bachelor’s degree in appropriate discipline with one to two years  progressive experience or an Associate’s degree with a minimum of four years’ experience in the field.  

Previous discharge planning, counseling, home care, and substance abuse treatment experience is desired.  Organizational management and leadership skills.  Oral and written communication skills, ability for problem solving and conflict management.

Pay Range: $21.94-$24.13

Employee salary is based on commensurate experience and other qualifications.
