In this role, you will provide comprehensive support and services to individuals facing crises, mental health challenges, substance use issues, and more. Your work will involve crisis intervention, diagnostic evaluations, counseling, and the development of personalized treatment plans to empower participants towards independence and long-term success.
Responsibilities:Crisis Intervention: Provide immediate support to individuals in emotional and/or physical distress.Diagnostic Evaluations: Conduct thorough assessments and refer clients to Licensed Clinicians for specialized treatment.Infection Control: Ensure strict adherence to infection control practices.Referrals: Connect participants with external programs for medical and psychological interventions.Drug Screening: Collect urine specimens for drug testing.Counseling: Offer counseling services to individuals with mental health issues, substance dependencies, and/or those who are homeless or HIV-positive.Patient Orientation: Guide new patients through treatment programs.Treatment Planning: Participate in the creation and implementation of individualized treatment plans, including individual and group counseling.Outreach: Identify potential clients and inform them about our agency's programs.Case Management: Develop, implement, and evaluate comprehensive treatment plans, maintaining regular contact with participants.Documentation: Complete necessary paperwork and ensure customer confidentiality.Advocacy: Liaise, refer, and advocate for participants with social service providers, emphasizing self-empowerment.Team Collaboration: Participate in team meetings, seminars, and trainings.Community Engagement: Collaborate with community providers on program content areas and present program information to various agencies monthly.Essential Skills:Education: Master's degree in social work or a related field with a license (LCSW, LADC, LMFT, LPC).Clinical Experience: Three years of related clinical experience with homelessness, mental health, or substance use.Technical Proficiency: Skilled in databases, MS Office, written reports, and case note maintenance.Organizational Skills: Strong organizational, motivational, time management, and interpersonal skills.Group Facilitation: Ability to facilitate groups and extensive experience in counseling.Cultural Competence: Experience working with participants from diverse ethnic and economic backgrounds.Knowledge Base: In-depth knowledge of Human Services, Justice system, Court systems, substance use, and target populations.Collaborative Spirit: Demonstrated ability to work collaboratively with other service providers.Additional Skills & Qualifications:Language Skills: Bilingual (Spanish) is a plus.Specialized Training: Training in crisis intervention, substance use, mental illness, domestic violence, or other relevant fields.Work Environment:This position operates in a professional office environment with standard office equipment. Work hours are Monday through Friday, 8 AM to 5 PM. The role may require occasional travel to various community sites and participation in community events.

Pay and Benefits
The pay range for this position is $24.00 - $34.00
