Adult Day Health Program RN -- $50/HOUR + Monday-Friday , 9am-3pm + 13 week assignemnt + Extensions and Temp-Perm option available + Job Description and Responsibilities below + Woonsocket RI Area Position Summary + To advocate for all medical needs of assigned program participants focusing on optimal health for all individuals. + Perform required nursing activities including medication administration, documentation, and direct support. Responsibilities + To provide direct support and training in relevant programareas to staff and participants. + Provide assistance as needed for program participants’ personal care needsincluding dining, toileting and transfers. + To coordinate medical services with each person’s personal physician/clinic inconjunction with residential providers and families. + To document medical needs and interventions in medical nursing notes andevaluative reports, including the participation in the Individual Support Planprocess and case conferences as required. + Oversee interns, students, and volunteers as assigned. + To obtain medical orders, medical reports and approval of treatment plans fromthe primary care physicians, administer medication and maintain a confidentialmedical file for each participant. + Assist in case record review process as assigned. + To provide CPR, First Aid and other pertinent training as trained and assigned. + To transport participants as assigned. + Participate in activities as a member of the Interdisciplinary Team asassigned. + Advise the Director and staff of any medical problems that may hinder aparticipant’s progress or that may be aggravated by activity at the program. + Complete preadmission review of medical history and needs of prospectiveprogram participants. + To foster the growth of individuals and to treat them with respect, dignity andin an age and culturally appropriate manner + To attend all staff meetings, clinical meetings and in-service trainings, whichare designated mandatory. + To be familiar with the regulations and statues governing and insuring thehuman rights of program participants. + Be familiar with, and assist in the implementation of, the Seven HillsFoundation Policies & Procedures, the site’s Program Specific Policies& Procedures and the regulations governing the program. + To perform additional duties as required or directed.
