Adult Day Program Asstistant
Ebenezer is looking for a Adult Day Program Assistant to join our team! This role is responsible to encourage socialization and assists in planning and implementation of programming needs of individual participants and groups and assist with personal care duties. Responsible for ongoing charting and case management. Responsible for preparation, implementation and cleanup of daily programs i.e. crafts, woodworking, games, music, etc. · Responsible for providing snacks and noon meal to participants. Set up and cut up food as necessary and clean up. · Responsible for daily menu planning and ordering of meals and snack supplies. · Perform routine personal care and grooming at the center (including baths). · Assist staff in developing monthly calendar. · Responsible for assisting identified participants with all toileting needs. · Promote and encourage individual care. · Communicate with staff the participants needs and individual plan of care. · Assist clients with mobility if needed and assist them if needed in getting to/from activities. · Observe, assess and intervene where needed with client’s health concerns. · Assist client/families with transportation related tasks. · Report any problems, progress and observations to the Director. Maintain monthly charting, quarterly care plans and health updates. · Document monthly health education, fire drills, blood pressures and weights for each participant. Responsible in maintaining clean areas. · Maintain safe area for participants. · Maintain adequate supplies for cleaning and caring for participants Required Current dirver's license in good standing 0 to 1 year of previous experience Preferred vocational /Technical training or Associate Degree 1 to 3 years of experience Windows/Microsoft previous experience
