Position: Adult Family Care Nurse (RN or LPN) Location: Wakefield, MA Pay rate: $35 per hour - Tiered pay rates based on years of service, pending performance evaluations Hours: Part-time, 12-16 hours per week, Monday to Friday with some flexibility based upon family needs This position will provide support both remotely and in-person. Summary & Responsibilities The AFC Nurse provides nursing case management for AFC Participants and Caregivers including development and evaluation of AFC Plan of Care, goal setting, training, advocacy, and referrals. + Conducting on-site and telehealth visits with each participant and caregiver in the qualified setting monthly or bi-monthly and as needed. + Developing, implementing, and reviewing the AFC plan of care. + Training, evaluating, and supervising AFC caregivers in conjunction with the case manager. + Completing nursing assessments and progress notes, ensuring the documentation is completed in a timely manner. + Submitting a semi-annual health status report to the member’s primary care physician and communicating with primary care physician as needed. + Assisting the participant in obtaining information and accessing other healthcare and community services. + Interfaces with other agencies to coordinate services. Qualifications + Registered Nurse or Licensed Practical Nurse with current Massachusetts license.
