Description and Responsibilities
The Adult Learning Center is a Day Habilitation program that serves 45 adults between the ages of 23-85 with a diagnosis of intellectual disabilities and some physical challenges. Services and support provided to participants include, but are not limited to occupational therapy, speech therapy, physical therapy, nursing oversight, functional skill development, communication, socialization, community membership, and self-advocacy. Staff at the Adult Learning Center implement structures and assist in maximizing participants involvement in activities.
The Licensed Practical Nurse will coordinate medical services with each individuals PCP, or medical clinic and complete nursing assessments and documentation, as required. They will also develop and implement Individual Service Plans with people served and other members of the interdisciplinary team and advise the program director and other Day Habilitation staff of any medical problems that may hinder an individual’s participation in Day Habilitation, or in a specific activity.
Other Key Responsibilities:
Administer medications according to physician’s orders. Accompany individuals served to medical visits as needed.Advocate for individuals served with physicians and other providers as necessary.Work in close collaboration with the RN to maintain medical records and proper documentation.Provide training on current medical issues, and procedures as needed including hygiene and health concerns. Qualifications Licensed Prractical Nurse in the Commonwealth of MA, required.Physical stamina and lifting individuals served is required.
Valid Drviers License and acceptable driving record, required.
