Advance Care Planning Specialist

Baptist Health is looking for an Advance Care Planning Specialist to join our team. This is a remote position that requires residency in KY or IN

In collaboration with Advance Care Planning (ACP) Organizational Faculty, assists with program development and system wide implementation of advance care planning by coordinating ACP activities, and identifying resources needed. Provides ACP expertise to produce quality outcomes and promote evidence-based practice in collaboration with the patient, family, physicians, other medical professionals and all applicable stakeholders. Assesses the needs of individuals seeking advance care planning assistance, providing a person-centered approach to advance care planning based on the individual’s health, readiness, fears, concerns and spiritual or cultural beliefs. Assists with training of facilitators and instructors under direction of Organizational Faculty. Aids in promoting advance care planning as a standard of care across the health system.

Essential Duties:

Work with patients to conduct advance care planning discussions, review documentation within the electronic medical record, and document conversations and completion of advance directives within the electronic medical record.

Contributes to a positive experience for patients and families through courteous telephone interactions and interview activities, accurate and expeditious routing, as well as referral to appropriate clinical staff when necessary.

Assists in coordinating outpatient and inpatient referrals to advance care planning.

Manages ACP data collection, prepares reports on findings, and provides interpretations and recommendations to ACP coordinators and care sites within Baptist Health.

Supports advance care planning facilitators to develop creative processes to proactively engage in advance care planning discussions at their care site.

Coordinates ongoing competency, education and training activities for new and existing advance care planning facilitators.

Contributes to the creation of, maintenance and improvement of community outreach and education.

Minimum Education, Experience, Training and Licensures Required

Bachelor’s degree graduate of an accredited undergraduate university/college in applicable field such as but not limited to nursing, social work, ethics, law, theology, bioethics.

Experience with advance care planning and/or involvement in system programs is desirable.

Prior use of electronic health records and other health care information systems desirable.

Current state license required if applicable (i.e., nurse or social worker). Training and certification in a healthcare related field, such as nurse, social worker, or chaplain preferred.

Advance Care Planning facilitator certification must obtain within 6 months
