As a key contributor toward that goal, the Regional AdvancedCare Coordinator (ACC) serves a pivotal role in the team in order to bring individualized care to the patient’s home as an alternative to hospitalization. The Regional ACC brings organizational, communication, and collaboration skills to our innovative team. The Regional ACC provides support to our clinical teams as we bring comprehensive care to patients and their families in an alternative setting. This position provides the opportunity to contribute to the creation and refinement of a novel care model while helping reduce hospital admissions, decrease ER/inpatient utilization, increase patient success, and increase the patient and provider experience. Weekends Only FT Position What You'll Do Office coordination for an assigned region for Advanced Care clinical and leadership teams. Facilitate exceptional patient and team member experience by coordinating day-to-day administrative operations and logistics. Coordinate with assigned market leadership and clinical teams Responsibilities include but are not limited to: Clinical care systems support Dashboard workflows EHR registration & other support platforms Insurance eligibility and authorization submission Remote patient monitoring set up Assist with data entry for program analytics Support clinical teams day-to-day Support exceptional patient experience Facilitate communication with key network partners Provide administrative support for all transitions of care for the patient Daily patient schedule creation Upload documents to EHR via secure messaging Monitor incoming faxes, upload documents and alert teams as appropriate Send outgoing faxes such as patient records Request patient records Other duties as assigned What You Need Competencies/Qualities Willingness to evolve as the system-of-care expands and evolves Exceptional prioritization skills Outstanding verbal and written communication skills Ability to flex between technology platforms Strategic and abstract thinker Passion for redefining healthcare Highly organized and task driven Work cooperatively, positively, and collaboratively with an interdisciplinary team Education and Experience Associates degree, or equivalent work experience Strongly encourage working toward bachelor’s degree 1-year minimum working in office administration or medical office Customer service experience Some healthcare experience preferred High technology proficiencies Who We Are DispatchHealth is redefining healthcare delivery through mobile and virtual healthcare.
