Apply Today! **Job Summary and Qualifications** The advanced clinical pharmacist practices the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. In addition, the advanced clinical pharmacist is expected to lead pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences and contributing to publications, and providing regular, formal education to pharmacy and interdisciplinary teams. The advanced clinical pharmacist is responsible for one or more specialty services within the hospital, such as infectious disease (ID), critical care, and pediatrics, among others. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the clinical pharmacist. + Leads committees responsible for establishing and implementing medication-related policies and procedures (e.g. Pharmacy & Therapeutics, Medication Safety, Antimicrobial Management Program) + Is primarily responsible for the direction of one or more service lines with respect to pharmacy service lines (e.g. cardiovascular, pediatrics, critical care, etc.) + Demonstrates leadership within the pharmacy department through a documented record of improvements in and contributions to pharmacy practice + Maintains ongoing commitment to advancing the profession in the area of pharmacy practice in which they serve + Provides regular, formal education to pharmacy and interdisciplinary teams in their field of expertise + Models, mentors, and encourages pharmacy students, residents, and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity + Performs all responsibilities of the clinical pharmacist as outlined below + Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervenes when orders are unclear or potentially unsafe or inappropriate + Interprets medication orders (verbal and written), transcribes to computerized patient medication profiles accurately, follows facility practices and procedures for documentation in the patient’s medical record and trouble shoots/clarifies Centralized Order Entry (COE) orders as appropriate + Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines + Evaluates Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate + Issues controlled substances to patient care areas and maintains records as required by law and follows corporate policies and procedures related to the dispensing and monitoring of controlled substances + Ensures safe, appropriate, evidence-based, and cost-effective medication therapies for patients according to established policies, procedures, and protocols + Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug- laboratory test interactions, allergies, appropriateness of drug and dose, indications and therapeutic duplications + Based on knowledge and expertise, develops, assists, implements, and executes all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy-directed dosing + Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed for drug-bug mismatch, de-escalation and duration of therapy + Reads, extracts, and interprets information in patient charts accurately + Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner, shares good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services and reviews medication related events that have occurred to prevent recurrences + Sustains the formulary by instituting drug therapy modifications for formulary compliance, SKU reduction and management of shortages as applicable + Evaluates and responds to patient-specific drug information questions from clinicians + Provides drug information to patients and their families and performs applicable medication reconciliation + Ensures CPW rules/alerts are reviewed and completed in a timely manner, utilizes Vigilanz/CPW to document clinical interventions and ensures communication for consults, documentation for the permanent medical record or for other disciplines occurs in the permanent electronic health record + Documents all required double checks per facility practice + Participates in the quality improvement and medication-use review activities of the department and collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned + Participates in rounds and/or direct follow up with providers. This may include interdisciplinary rounds, rounds with individual providers, onsite/in person follow up with providers, and via phone or written communication based on urgency/ability to meet with provider in person. + Participates in the nursing, physician, or ancillary team onboarding and staff meetings as appropriate to explain clinical pharmacy services and educate on new drugs or updated clinical guidelines + Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel + Maintains current pharmacist licensure, and maintains competence required for current job title/position. Pursues board certification or other training/certification for clinical area of expertise based on job role + Participates in local and/or national pharmacy organizations, and pursues research, publication and speaking opportunities + Attends orientation, education, and training programs and reviews literature and other materials pertinent to the practice of pharmacy + Maintains strict confidentiality of patient, visitor, and employee information and complies with HIPAA Privacy Rule standards + Is able to work with automated systems. Is granted Automated Dispensing System (ASDS-Pyxis) /Meditech access and privileges.
