Advanced Practice Nurse

Conducts medication re-evaluations for consumers who have been seen by the psychiatrist. Aids in formulating diagnostic impressions based on DSM-5 criteria in collaboration with the psychiatrist and other treatment members. May practice in the role of therapist, providing therapy, as needed. Can write prescriptions for non-controlled substances. Will be in verbal and/or electronic contact with the supervisory psychiatrist. Attends meetings as required. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. The individual demonstrates knowledge of the principles of growth and development appropriate to the consumer population served. He or she must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group.

 

The Crisis Diversion Home provides recovery-oriented temporary transitional housing for up to 5 individuals, up to 30 days, within a 24-hour supervised environment. Two staff are on site at all times to provide therapeutic and social supports to consumers in crisis referred from Crisis Receiving Stabilization Centers and Mobile Crisis Outreach Response Teams.
