Posting Summary Rutgers, the State University of New Jersey, is seeking a Advanced Practice Nurse (APN), Opioid Response Team for NJMS-Emergency Medicine.
The Advanced Practice Nurse (APN), provides substance use services to patients managed by the opioid response team (ORT) and the Comprehensive Addiction Resources & Education (CARE) Center at Rutgers New Jersey Medical School. The APN will work with a mobile ORT team to coordinate and link patients to medications for opioid use disorder (MOUD) and harm reduction services in the prehospital setting. The APN will also provide clinical care at the CARE Center for ongoing MOUD treatment, collaborate with pharmacies to improve patient adherence to long-acting MOUD, and educate EMS, first responders, and community groups on opioid use disorder, naloxone, and MOUD. Additionally, the APN will assist with phone consultations for providers seeking guidance on MOUD initiation and maintenance.
Among the key duties of the position are the following: Conduct fieldwork to provide direct clinical care to patients managed by the Opioid Response Team (ORT) in the prehospital setting, which will involve riding in ambulances and visiting patient homes or places of residence (e.g. shelters). Conduct clinical shifts at the Comprehensive Addiction Resources & Education (CARE) Center, treating patients referred for ongoing MOUD (Medications for Opioid Use Disorder) treatment. Assess and manage patients with opioid use disorder (OUD), initiating and maintaining MOUD. Collaborate with ORT members, including the Community Paramedic, Community Outreach Coordinator, Peer Navigator, and Program Manager, to coordinate patient care. Facilitate seamless transitions of care from the prehospital setting to ongoing outpatient treatment at the CARE Center. Work closely with the University Hospital Ambulatory Care Pharmacy and neighborhood pharmacies to facilitate access to MOUD, particularly long-acting MOUD formulations, for improved patient adherence.
