Advanced Practice Nurse - Telephonic Medicine Center - PRN, Oncall

Job Summary:
The Telephonic Medicine Center (TMC) Nurse Practitioner (NP) practices professional nursing in accordance with accepted practice standards, by performing independent nursing functions and delegated medical functions in the Telephonic Medicine Center by handling a variety of requests from internal and external KFHP-CO sources and ensures that members are connected to the highest, most appropriate level of care needed to resolve their issue. Provides care delivery in a telehealth setting through various means including screens calls, triages resource, provides support, and collaborating with all care resources in KFHP-CO.

Essential Responsibilities:

This position knows and complies with all KP (Kaiser Permanente) quality, safety, and emergency policies and procedures.
Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership.
Ensures patient safety in the preparation and provisioning of care related to medications, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs.
Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.
Handles a variety of requests from internal and external points seeking advice on the most appropriate level of care needed to care for KFHP-CO members.
Assesses patients' needs based on information provided via virtual connections from member contact points or members.
Maintains knowledge of KP resources and assigns appropriate level of care based on assessment.
Considers evidence-based practice, patient preference, and other resource information to deliver culturally sensitive health, and enable members with informed decision making.
Provides Medical advice and/or manages appropriate cases.
Escalates urgent/emergent situations.
Provides member care in a safe, efficient, and cost-effective manner.
Educates and counsels healthcare teams and member on care plans and treatment options.
Clearly and concisely documents all interactions in KP electronic medical record and other KP systems.
Assists with arranging transportation to KP venues and provides call-ahead notification.
Collaborate with other members of the TMC and KP resources to provide care congruent with accepted standards of practice.
Obtains medical history through, conveyed information, member interaction, and chart review and develops assessment, plan, including and not limited to prescribing treatment plans and medications or other needs prior to disposition.   
Understands the need to review lab and other results, including critical results and intervenes appropriately to support member care needs during usual and atypical business hours.
As a TMC team member, assists in various situations to deliver high quality, affordable care.
Attends mandatory department meetings and performs miscellaneous duties as assigned by Supervisor/Manager.
Contributes to team patient care goals and standards of care.
Participates in the training and/or mentoring of new employees and colleagues.
Participates in safety and quality assurance activities including the peer review process.
Continuously understands and complies with relevant (federal, state, and local rules and regulations and organizational policies including licensure and certification.
Ability to deliver care in telehealth setting using various communication methods and technologies.
Ability to bend and sit for prolonged periods.
Performs other duties as assigned by manager/supervisor.
Basic Qualifications:
Experience

Minimum of three (3) years of clinical Nurse Practitioner practice treating all ages in an Urgent Care, Acute Care Hospital, Emergency Department or Family medicine setting (New Graduates not considered).

Education

Graduate degree in nursing
Graduation from an accredited graduate nursing program approved by the Colorado State Board of Nursing.

License, Certification, Registration

Advanced Practice Nurse Registration (Colorado) required at hire


APN Prescriptive Authority License (Colorado) required at hire


Registered Nurse License (Colorado) required at hire


Advanced Cardiac Life Support within 3 months of hire


National Provider Identifier required at hire


Basic Life Support within 3 months of hire


Pediatric Advanced Life Support within 3 months of hire


Nurse Practitioner National Certificate required at hire


Drug Enforcement Administration Registration required at hire


Additional Requirements:

Ability to work independently such as experience with limited on-site supervision or experience with significant self-directed accountabilities.
Ability to work with a multidisciplinary team; demonstrated customer service skills, interpersonal skills, and communication skills.
Must have full prescriptive authority.
Well versed in or willing to learn resources specific to KPs integrated care delivery system (including primary care, urgent care, virtual and other on demand care delivery options) to ensure members receive care in the most appropriate setting.
Must be able to work in a Labor/Management Partnership environment.
Expected to work any shift in a 24/7/365 including nights, holidays, and weekends.

Preferred Qualifications:

Recent experience as an APN in an Emergency Department, Urgent Care, or Acute Care Hospital setting.
