Overview ADVANCED PRACTICE PROVIDER - HOSPITALIST DEPARTMENT - MEDICINE St. Catherine of Siena Hospital is a proud member of Catholic Health and has served the residents of the Smithtown area for more than 50 years. St. Catherine's is located on the north shore of Long Island in Smithtown. The campus is comprised of a 296-bed not-for-profit community hospital; a 240-bed nursing and rehabilitation care center; and a 60,000 square foot medical office building. Job Details Advanced Practice Provider Medicine Team Summary: Work collaboratively with physicians, nurses, and other healthcare professionals to provide comprehensive care to hospitalized patients Experience in hospital medicine and a strong commitment to delivering high-quality patient care Advanced Practice Provider Medicine Team Responsibilities: Conduct thorough patient evaluations and physical examinations Develop and implement patient care plans in collaboration with attending physicians Order and interpret diagnostic tests, including laboratory tests, imaging studies, and other relevant tests Perform midline catheters (training provided) and wound care as needed Monitor patient progress, adjust treatment plans, and ensure continuity of care Provide patient and family education regarding diagnoses, treatment options, and discharge planning Participate in daily rounds with the hospitalist team to review patient status and plan for ongoing care Document all patient interactions, diagnoses, and treatments accurately and in a timely manner in the electronic health record (EHR) Assist with discharge planning and coordinate follow-up care with outpatient providers Respond to emergencies and urgent medical situations as required Adhere to hospital policies, procedures, and best practices to ensure patient safety and compliance with regulatory standards Engage in continuing education and professional development to stay current with advances in hospital medicine Respond to medical emergencies and work collaboratively with Attending Support throughput, length of stay and patient experience, support physician staff Advanced Practice Provider Qualifications: Education: BS or MS. degree in accredited Physician Assistant Program or MSN in accredited Nurse Practitioner program Experience/skills: 2 years’ experience preferred Licensure: Current registration with NY Education Dept. as Physician Assistant. NCCPA Certification. ACLS, BLS Salary Range USD $66.55 - USD $90.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
