Job Details Responsibilities Communicate with the patient, family and interdisciplinary team to ensure the care being provided is consistent with patient/family goals. Provide extensive and direct specialized palliative care to adult and geriatric patients through a consult service. The role requires the ability to provide care based on physical, psychosocial, spiritual and emotional assessments. Assist the patient/family with completing advanced directives and establishing GOC which can potentially reduce the utilization of advanced medical care in the SDU/ICU units. Order and titrate medications to relieve symptom burden using evidence based guidelines. Educate patients, families and staff regarding prognosis, what to expect at EOL and side effects of medications. Transition comfort model of care and initiating and overseeing a compassionate wean from life support. Become a member of the ethics committee and become comfortable in discussing risk vs benefit and the relevance of Ethical Religious directives to assist the patient/health care proxy with medical decision making. Advocate for the advancement of the nursing profession. Provide care for patients who are living with complex medical conditions and life limiting illness. Requirements RN Degree and NP degree Licensure: New York Registered Nurse (RN) License & Registration ACLS and BLS certification from the American Heart Association Obtain Palliative Care certification within approximately 2 years of employment Salary Range USD $63.38 - USD $89.34 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
