Community Health Network has an excellent opportunity for a board-certified, family medicine trained advanced practice provider to work in our Retail locations. Community Health Network's Community Clinics put high quality, convenient primary and acute care throughout all regions of Indianapolis and the surrounding suburbs.
Job Duties
Advanced Practice Provider will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Minimum two to five years of family medicine or urgent care experience as an advanced practice provider firmly required; this is an independent position requiring a strong level of autonomy

Educational background consistent with requirements for licensure and certification with the State of Indiana
