About This Career Opportunity
Health Care Coordinator:
Responsible to the Resident Care Director for direct resident care and administration of medications in accordance with the care plan. Trains the Medication Assistants/Technicians and provides oversight to the Medication Program, including monitoring the accuracy of the medication distribution system and ensuring that medications are dispensed per physician orders. Ensures necessary documentation is being performed to meet regulatory requirements and adhere to policy and procedure. Assists with admission and discharges, including performing admissions valuation, assuring that medical records are completed and medical orders confirmed. Performs assessments of residents when there is a change of conditions and follows-up with the appropriate medical team for intervention and notification of family members. Works with the Resident Services Director and the Resident Assistants to ensure the individual resident care needs are being met.Overview

Responds to Resident complaints and inquiries in a caring and compassionate manner. Takes initiative to follow through until service or problem is completed and resolved. Evaluates residents for changes in conditions. Reports findings of evaluation to the appropriate physician and Resident Services Director and implements and coordinates care with other members of the health team, including participating in family care conferences. Completes end of shift reviewMedication Distribution Monitoring
Monitors the accuracy of the medication distribution system. Ensures that medications are dispensed per physician orders. Ensures necessary documentation is being performed to meet regulatory requirements and adhere to policy and procedure. Provides in-service training to med assistants. Complete weekly in-house medication audits. Completes Pharmacist audit follow ups. Completes PRN conversion checks. Responds to complicated med questions/orders.Daily Rounds
Makes daily rounds to monitor the care provided and ensure that resident care is performed in keeping with the service plan, and that levels of grooming are appropriate. Monitors that resident's food intake is appropriate and that all documentation is being performed per unit routine. Performs any treatments requiring a license.Admissions & Discharges
Assists with admission and discharges, including performing admissions valuation, assuring that medical records are completed and medical orders confirmed. Records all pertinent information regarding changes in the residents’ status in the medical record. Works in collaboration with other staff to coordinate assessments for admissions and give daily status reports of residents. Completes Resident assessments and service plans.Charting
Completes charting accurately and timely following resident care interactions.Documentation Standards
Meets current documentation standards and policies and successfully documents in the E.H.R. system. Notes and carries out physician orders in a timely manner.Qualifications – Position Requirements
At least 18 years of age. Cleared Criminal Record clearance through the Department of Social Services prior to presence in the community. Health screening and TB test results not older than 6 months. Current First Aid Training and Certifications as per Title 22. 3-5 years in long term care, home care or related field preferred. Current California RN or LVN license required. Minimum 2 years’ experience in long term care, home care or related field. Acute/skilled nursing community experience preferred.Skills and Attributes
Sensitivity and knowledge of working with older adults and the aging process. Ability to identify need, develop, and manage programs and systems simultaneously. Knowledge about dementia highly desirable. Excellent written and verbal communication skills. Knowledge of aging process and implications of aging in a social setting. Must understand health care environment and the medical context for older adults. Ability to communicate professionally with residents, families, peers and employees in a multi-cultural environment. Attention to detail. Working knowledge of computers and typing, sufficient to be successful with computerized Point of Care and other systems.Compensation and Benefits
