Our Resident Care Coordinator:


Coordinates and organizes the medication program with the Health Care Coordinator Provides coordination and leadership in the delivery of care to all residents in Assisted Living and Memory Care, as appropriate. Ensures proper staffing through completion of the staff schedule and management of vacations and call-offs. Ensures that medical records are completed and in compliance with Title 22 and Federal Regulations, to include, but not limited to: Initial Advance Directives, Resident Rights information, and evaluations required.


What you need to succeed:


Current First Aid Training and Certifications as per Title 22. Working knowledge of computers and typing, sufficient enough to be successful with computerized Point of Care and other systems. Attention to detail. Must possess effective English verbal and written communication skills with the ability to successfully work in a multi-cultural environment. Minimum 6 months experience in long term care, home care or related field.
