The Case Manager will assess and determine appropriate admissions to Ashley Woods and provide necessary support for residents to maintain independence and personal choice. The case manager will manage all aspects of clinical service programs at Ashley Woods in conjunction with the Resident Care Manager. The case manager will also be the administrator of the Memory Care Unit and work with the Executive Director 


Full-Time, Days 

Starting Salary: $63,000


ESSENTIAL JOB FUNCTIONS

Participate in strategic planning with a concentration in Memory Care.Oversee the functionality of Memory Care and provide support and ideas for improvement.Assure that each resident meets the retention standards for the level of care offered in the assisted living residence (ALR) e.g. enriched, enhanced or special needs. Conduct initial functional assessment and client interview for applicants.Meet with all applicants and families; conduct follow-up tours as needed and document interviews.Discuss a potential plan of care with the resident care director to assure the resident is being appropriately placed.Recommend admission to executive director for final approval, if needed.Participate in marketing Ashley Woods by providing tours, as needed, and assisting with complex inquiries as needed.Assist with apartment selection as available.Update senior life specialist regarding upcoming vacancies.Review necessary admission paperwork (DOH-3122, DOH-4397) and resident handbook with the resident and/or responsible party. Complete the admission agreement with the resident and/ or responsible party. Obtain POA, HCP and contact information as necessary.Orient new residents to Ashley Woods.Coordinate the completion of Individual Service Plan (ISP) and review biannually. Evaluate each resident annually, using functional assessment tool. Complete functional assessments on residents experiencing changes in condition.Encourage residents to maintain independence and participate in activities.Maintain records documenting the case management needs of each resident and case management activities performed.Coordinate care with other providers.Provide family with support as needed and keep updated as appropriate on resident’s status.Coordinate family/resident meetings.Participate in and facilitate resident association meetings and family forum.Communicate all pertinent information to other staff.Assist residents and families in planning and arranging alternative care setting, as desired/ needed.Communicate with families, physicians, and other care providers in emergency discharge situations.Arrange transportation for resident, if necessary.Conduct in-services e.g., residents’ rights for Ashley Woods staff as needed.May be assigned on-call duty on a rotational basis to handle resident or program emergencies, as needed.


Requirements

QUALIFICATIONS: Excellent oral and written communication skills. Demonstrated flexibility in problem solving. Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors, and the general public.


EDUCATION: A Master’s degree in social work from accredited college or university is preferred, or Bachelor’s degree in social work or related field.


EXPERIENCE: At least two-year experience working with older adults.
